Interpreting wrongly

I have faced it multiple times that when an Interviewer is talking and is asking some scenario based questions, it is interpreting words wrongly or is not able to cope up with the story properly which resulted in missing out the main question.

Also, it is visible over teams which goes against of what you have advertised.
